Need help finding a web app! Options

Hello everyone,

We need help finding a web app which can do the following:

The scenario is that our rental home owners will order a video from us. Once they order the video, an autoresponder will go out to them with a questionnaire to fill out.

We need a web app that does the following:

1) Allow us to customize the interface with our logo, and simple instructions about the information that we need from them.

2) Allows our customer to upload their photos easily, and add descriptions for each photo (preferably drag and drop, but don't know what's out there)

3) Has a secure user name / password for EACH user (they can sign in, doesn't need to be auto-generated, but we need to receive notice of when something is uploaded. So, for example, uploading to FLICKR wouldn't work because we would need to download each file, and upload the finished video when we're done. So it needs to be two-way

4) We're fine on paying for a service, the biggest thing is that it has to be super easy to use for a variety of user skill sets. Most rental property owners aren't super tech-savvy.

Any help or suggestions would be much appreciated!
