Can you please update your version of Hijackthis
Open Hijackthis>>Config>>Misc Tools>>Check for updates Online
If for some reason it won't update you can download the latest version from

Save it to your C:\My Downloads folder, allow it to overwrite the old version if prompted

Please follow these instructions if possible
I'm going to ask you to modify the registry
Beforehand please make a fresh Restore point
Go to START>>All Programs>>Accessories>>System Tools>>System Restore
Create a Restore point
Name it and click Create
This is just to ensure you have a backup from this point on

This is supplied by Trend Micro
# Open Registry Editor. Click Start>Run, type REGEDIT, then press Enter.
# In the left panel, expand(+) the following
+HKEY_CURRENT_USER
+Software
+Microsoft
+Internet Explorer
+Desktop
+Components
# Still in the left panel, locate and Right click on and delete the subkey:
0
# Close Registry Editor.

Using Windows Explorer, locate and delete the following files:
Do a search for the bolded files
• %Windows%\desktop.html >>you may also find and entry in your Web folder
• %Windows%\SSICO.ICO
• %Root%\Documents and Settings\<current user>\Desktop\! Protect Your Data.url
• %Root%\Documents and Settings\<current user>\Favorites\! Smart Security.url
• %Root%\Documents and Settings\<current user>\Recent\! Smart Security.url
• %Root%\Documents and Settings\<current user>\Start Menu\! Secure Yourself.url

After that is done, Restart your computer

Before posting back with a fresh hijackthis log
I see you may be controlling startup entries with msconfig.
Not that there's anything wrong with that, but you may be unknowingly be hiding Malware that you are not aware of
Can you please go to START>>>RUN>>Type in msconfig and hit Enter or OK
Click the Normal Startup
Apply it and close out and Restart your computer

Post back with a fresh hijackthis log from version 1.99

Your last log looks good

You may however want to do another scan with hijackthis and put a check next to this entry

Beforehand enter your taskmanager and end process on it

When all other windows are closed, including this one, click the FIX CHECKED button

Restart your computer

Alcxmntr.exe is related to Realteks AC97 Audio

If you have any problems with your audio afterwards, simply restore that entry with Hijackthis from the backups
I can't see you having problems disabling it however, and anything that reports back
I consider to be just as bad as spyware
