Internet Connection

Hello,
My internet conections shutdows every 5-10 min and comes back in about 1 min, why is that? i scaned with anti virus,antispyware,even did windows check with chkdsk utillity. My internet conection is dialup. Modem is brand new ( both 1 week ago ) . So can you help me? Heres the HJT log:
